Hey everyone, I'm Miyavi from Small Wins!
Today, I'm going to share an incredibly useful tip with you: creating survey forms and directly linking the results to your Notion database.
Excited? Let's dive in!
Heads up: This letter includes affiliate links. If you click and purchase, I may receive a small commission at no extra cost to you. I only recommend tools I personally vetted.
I used to rely on the simplest tool out there - Google Forms.
But here's the catch: the results would only go to a Google spreadsheet.
For someone like me, whose main workspace is Notion, this was a real headache.
I searched for several form tools, but most required a paid subscription.
Then, I stumbled upon NoteForms. (And boy, was I in for a surprise!)
NoteForms' free version is a game-changer.
It fulfills my need to create surveys and send results straight to my Notion database.
And the setup? Super easy!
No more copy-pasting between Google Sheets and Notion. (Why didn't I find this sooner?)
Before we jump into the tutorial, let's get to know NoteForms a bit better.
Ready?
What's NoteForms?
Noteforms is a form tool tailor-made for Notion.
It lets you create all sorts of forms - surveys, sign-up sheets, feedback forms - you name it!
But here's where it really shines: it seamlessly sends collected data directly to your Notion database.
And guess who trusts NoteForms? Big names like Harvard University, DBS Bank, Hyundai, Yale University, and Sciences Po Paris.
(If it's good enough for them, it's good enough for us, right?)
NoteForms' Standout Features
1. Seamless integration with Notion databases
Say goodbye to manual data entry! You can:
- Create a form and let NoteForms build a corresponding database in Notion, or
- Generate a NoteForms form from an existing Notion database
P.S. Editing field titles in Noteforms is a Pro feature, but there's a workaround. Simply create a Notion database with all the properties (questions) you need and generate a NoteForms form.
2. Clean, intuitive interface
Minimalists, rejoice! NoteForms' interface is clean, tidy, and fuss-free.
No need to decode complex designs - you'll be a pro in no time!
3. Diverse question types
NoteForms follows Notion's database properties.
Whatever your database can handle, your survey can ask.
Date? Multi-select? File upload? You've got it all!
4. Conditional logic (Pro plan)
Make your forms smarter with "if this, then that" logic.
It's like having a conversation with your respondents!
5. Data analysis (Pro plan)
NoteForms doesn't just collect data - it helps you make sense of it.
Quick insights at your fingertips!
6. Share form responses (or any Notion database)
Beautify your results, choose what to display, and share with colleagues or partners. Want to see an example? Visit this page.

Tutorial: How to Connect NoteForms and Notion
Ready to dive in? Let's connect NoteForms and Notion!
Step 1 | Head over to the NoteForms website and click "Create form for free".
Step 2 | Create an account.
Step 3 | Click "Create form".
Step 4 | Hit "Login with Notion".
Step 5 | Choose your Notion account in the top right corner, then click "Select Pages".
Step 6 | Tick the pages you want to give NoteForms access to, then click "Allow Access".
Step 7 | Do you already have a Notion database for form responses?
- Yes: Responses will automatically import to your existing database.
- No: Noteforms will create a new database in the Notion page you just selected.
❗Pro tip: Editing field titles in Noteforms is a Pro feature. I recommend setting up your database and fields in Notion first for a smoother experience!
Step 8 | If you chose "No" in the previous step, NoteForms will ask which Notion page to add the new database to.
Step 9 | Welcome to the form editing interface!
Step 10 | Remember, free users can't edit field names in NoteForms. The workaround? Adjust your database properties in Notion, then come back to NoteForms and click "Refresh Form Structure".
After updating, new fields are hidden by default. Just click the eye icon to unhide them.
Step 12 | All set? Click "Publish form" in the top right corner.
And voila! You've just mastered the NoteForms-Notion connection!
I hope this guide helps you seamlessly incorporate Noteforms and Notion into your daily life and work.
